Forms and Applications

In order to improve efficiency (and to save our customers time) you can now download the following forms and applications*.  Once you have printed the documents you can bring them to Madisonville City Hall at 400 College Street, Madisonville, TN.

Application, Residential City Service, Water & Gas – If you wish to add Gas or Water Service to your location, please print and complete the form.  Next, you must deliver to Madisonville City Hall and pay for the Activation cost and other applicable charges.

Agenda Request Form – To be placed on the agenda (approval required), complete the Agenda Request Form.  This form must completed and returned to the City Hall seven days prior to the first Monday of each month.

CHRISTMAS PARADE APP – To enter a float into the Madisonville Kiwanis Club Christmas Parade, please complete the application and return to the City Hall.

The Sewer Rates are the same as the Water Rates except that tax is added to water and not to sewer.

Sewer is only available inside the city.
There is no separate rates for commercial or small industry.

Disclaimer:  If you pay after office closing hours, your payment will not be processed until the next morning and will be treated as a late payment..  As a result, any disconnect/reconnect fees may be incurred from the late payment.

Instructions for paying online.

Utility – Madisonville, City of

To pay your Utility bills choose Tennessee for the state, then Madisonville, City of for the County/City.  Finally for Payment choose Utility.

Then fill out the remaining forms to submit your payment and a payment receipt will be emailed to you.

Property Tax – Madisonville Tax, City of  (NOTICE THIS IS NOT THE SAME AS UTILITY)

To pay your Property Tax choose Tennessee for the state, then Madisonville Tax, City of for the County/City.  Finally for Payment choose Tax.

Then fill out the remaining forms to submit your payment and a payment receipt will be emailed to you.

If you still want to see the awesome staff for the City of Madisonvile, we will be happy to take your payments at Madisonville City Hall.

Increasing our efficiency with new meters

Over the past 2 months, the City of Madisonville has upgraded the water and gas meters to digital meters that allow drive-by reading capabilities.  The new meters allow our staff to drive into your neighborhood to quickly and accurately gather the readings from our citizens’ meters.

These new meters are from Neptune.   Here some details from their site

All Neptune water meters are automatic meter reading (AMR) and advanced metering infrastructure (AMI) capable.
